PERIODONTAL EXPOSURE SURGERY OF A MAXILLARY CANINE INCLUDED IN ORTHODONTICS

Summary:

Between January 2010 and December 2022, in the context of private periodontology practice, 1,363 included teeth were exposed for orthodontic purposes. The maxillary canines accounted for almost 75% (1012) of all teeth, while the canine mandibular group accounted for 5.06% (69).
 
This decidedly clinical presentation will address the various inclusion situations, from the simplest to the most extreme, and a surgical decision tree will be proposed.
The different periodontal exposure surgeries, intended to help the orthodontist place the impacted tooth with all periodontal tissues and the best aesthetic integration, will be illustrated.

PREVENTIVE TREATMENT OF INCLUSIONS AND CANINE RETENTIONS

Summary:

Maxillary canines are the second most frequently included teeth, with a prevalence rate of 1% to 3%. Palatal inclusion is more common in the Caucasian population. When left untreated, impacted maxillary canines can interfere with the alignment of adjacent teeth, reduce arch length, increase the risk of follicular cyst formation, and lead to recurrent infections. They can also cause external root resorption of adjacent teeth, as well as irreversible damage that may result in tooth loss.

Through various clinical cases, we will develop the different prophylactic measures to optimize the outcome of permanent canines:

– Is there a preventive approach to this inclusion? :

o What are the first clinical signs;
o Radiological signs and the appropriate implications.

– What preventive measures and timing are involved? :

o Transversal expansion / cross-bone (disjunction);
o Distalization of the lateral segments ;
o Extraction des canines de lait.

– The right therapeutic approach to inclusion:

o Dégagement chirurgical avant ou après la création de place ;
o Le contrôle du torque dans les zones adjacentes à l’inclusion ;
o Que faire en cas de résorption `

Cette conférence met en lumière l’intégration de l’imagerie 3D dans la démarche
diagnostique et thérapeutique, en s’appuyant sur le concept novateur du jumeau
virtuel. Elle explore également l’intérêt de la superposition d’images pour le suivi,
l’évaluation et l’optimisation des traitements.

The virtual twin: a digital turning point in ODF
o Il s’agit d’une reconstruction numérique tridimensionnelle du patient,
combinant les données issues du CBCT, des empreintes optiques et de la
photographie clinique.
o Ce modèle permet une analyse exhaustive, une simulation précise des
possibilités de traitements, et une communication facilitée avec le
patient et les autres praticiens.

2. 3D imaging: an advanced analysis tool
o Grâce à l’imagerie volumique, les praticiens peuvent explorer avec
précision les structures osseuses, dentaires et faciales.

CONSIDERATIONS REGARDING RADICULAR RESORPTION OF TEETH ADJACENT TO THE INCLUDED CANINE.

Summary:

Canines are frequently affected by eruption disorders, which may have consequences for the roots of the lateral incisors that serve as a guide during their eruption. It is essential to understand the processes of root resorption, to establish appropriate differential diagnoses, and to choose the appropriate therapeutic approach, while taking into account the impact on endodontics.
