In young patients, early periodontal care is based on a true alliance between orthodontist and periodontist. Mucosal frena and bands, often dismissed as trivial, can interfere with oral hygiene, promote gingival recessions, and compromise orthodontic stability. Managing them requires a shared approach to choose the most appropriate time to intervene and to avoid any aesthetic or functional sequelae.
At the same time, the recognition of a thin periodontal phenotype in the mandibular anterior region raises the question of whether tissue reinforcement may be needed before, during, or after orthodontic treatment.
In some cases, simple orthodontic correction can, by eliminating the “wire syndrome,” be sufficient to stabilize the tissues and make grafting unnecessary. This integrated approach illustrates how essential interdisciplinary collaboration is to preserve the tissue “capital,” ensure long-term stability, and guide our therapeutic choices. When should we intervene? And at what point? The analysis of clinical cases, supported by the literature, will provide elements of answer.

Aligner orthodontics has revolutionized the management of many malocclusions through an aesthetic, comfortable, and personalized approach. However, its indication in patients presenting with periodontal disease remains a matter of debate.
This conference offers an analysis based on scientific literature and clinical experience in order to assess the potential benefits and risks of aligner treatment in this particular context. The criteria for selecting patients, the essential biological precautions, as well as the collaboration protocols with periodontists will be discussed.
The aim is to determine whether, and under what conditions, aligner orthodontics can represent a reliable and safe treatment option for periodontal patients.
